After Your Grant

Congratulations on your grant. We’re proud to support the work your organization is doing in Fond du Lac County and our affiliate communities, and we look forward to hearing how it goes.

A Fond du Lac County nonprofit program in action, supported by a Foundation grant

This page explains the grant evaluation, which every grantee completes once the funded program has run, and shares how to tell the community about your grant. If you have questions at any point, please reach out to our grants staff.

Yourgrant evaluation

Once the funded program has run, nonprofits must complete an evaluation form describing how the grant was used and how it positively impacted the community. Our grants staff include the link to the form with the letter that accompanies your grant check, and the form is also available on this page.

To come: link to the grant evaluation form, to be added during implementation

The grant evaluation is a required step to be eligible for future grant opportunities. It is also part of the summary the Board of Directors sees the next time you apply, so it’s a chance to show what your program accomplished.

To come: evaluation timing policy. The current website’s one-month deadline and late-report language are not carried forward; what, if anything, to publish about timing is to be confirmed with the Executive Director during implementation

What the evaluation covers

  • How the grant was used. How the grant was used. What the grant paid for in your program.
  • What resulted. What resulted. How the program positively impacted the community and the people it served.
  • Photos, if you have them. Photos, if you have them. Pictures of the funded program help us share your story.
